HIPAA Notice of Privacy Practices
The Health Insurance Portability and Accountability Act (HIPAA) of 1996 establishes federal standards protecting sensitive health information from disclosure without patient’s consent. Your health care provider and health plan must give you a notice that tells you how they may use and share your health information. It must also include your health privacy rights. In most cases, you should receive the notice on your first visit to a provider or in the mail from your health plan. You can also ask for a copy at any time. The Department of Communication Sciences and Disorders and the UNCG Speech and Hearing Center comply with applicable Federal civil rights laws and does not tolerate discrimination against anyone in our UNC Greensboro community (students, patients, caregivers, faculty, and/or staff) on the basis of race, ethnicity, sex, gender identity/gender expression, sexual orientation, age, religion, national origin (ancestry), disability, culture, language, dialect, marital status, or military status.
